I certify that I am either (1) an adult age 18 or older agreeing on my own behalf, or (2) the parent or legal guardian of the minor contestant and I consent on the contestant’s behalf to participate in the Taulelle Concessions Corndog Eating Contest.
I understand that participation involves inherent risks, including but not limited to choking, gagging, vomiting, aspiration, allergic reaction, illness, serious injury, or death. I represent that I am, or the minor contestant is, physically able to participate and will stop immediately if unwell or if directed to stop.
In exchange for being allowed to participate, I voluntarily assume all risks and, on behalf of myself and/or the minor contestant, release and hold harmless the Sioux Empire Fair Association, Inc., Taulelle Concessions, and their organizers, judges, volunteers, venue representatives, agents, and affiliates from any claims, liabilities, damages, costs, or expenses arising from or related to participation, except to the extent caused by gross negligence or willful misconduct.
